Youth 3-on-3 Street Hockey Tournament
Complete the following form to register for the 3-on-3 Street Hockey Tournament (ages 15 and under) as part of the Chicago Blackhawks Training Camp Festival. The event is scheduled for Saturday, Sept. 22 at the Blackhawks Training Camp Festival 2012. Hurry - space is limited!

Tournament Highlights:
- $50 entry fee per team (Limit 6 per team)
- All participants receive a ticket to the Training Camp Festival and a t-shirt
- Watch the Blackhawks practice!
- Free parking
Three Divisions: 7-9 years old; 10-12 years old; and 13-15 years old
To register, fill out the below form and submit it by September 1, 2012. Teams may be placed on a waitlist due to availability.

In addition, free hockey clinics for children will run from 10 a.m.–1 p.m. It’s all part of Hockey Weekend Across Illinois where the joy of playing hockey is introduced to kids.
