1. NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING WITH AN ENTRY. The Meijer Hockey 101 Sweepstakes is subject to all applicable federal, state and local laws and regulations. Void where prohibited or restricted by law.

2. SWEEPSTAKES PERIOD: This Sweepstakes starts at 12:00 a.m. Central Standard Time (CST) on February 22, 2009, and ends at 11:59 p.m. Daylight Savings Time (DST)) on March 21, 2009 (the “Sweepstakes Period”).

3. HOW TO ENTER: Complete an official entry form at any participating Meijer location, while supplies last. One entry per person per visit. You can also mail in an entry by printing on a plain piece of 3” x 5” paper your complete name, address, age and daytime and evening phone number, and mailing the entry to: Meijer, Hockey 101 at Meijer, P. O. Box 1953 Grand Rapids, MI 49501. If you do not have a telephone, indicate “No Phone.” No entries will be accepted if received before or after the Sweepstakes Period. Meijer, Inc. (“Sponsor”) and Chicago Blackhawk Hockey Team, Inc. (“Chicago Blackhawks”), and their respective agents, are not responsible for late, lost, damaged, incomplete or misdirected entries. Entries which violate these rules will be disqualified. Entries become the property of the Chicago Blackhawks and will not be acknowledged or returned. No mechanically reproduced entries permitted. Illegible/incomplete entries are void.

4. WINNER SELECTION: Twenty-five (25) winners will be selected by random drawing from all eligible entries received. Each winner will be notified by telephone or U.S. Mail after March 21, 2009. In the event a winner cannot be contacted, declines the prize, or cannot accept a prize at the date and time it is offered, a random drawing will re-occur to replace the prior winner. Odds of winning will depend upon the total number of eligible entries received. Winner and guest will be required to sign an affidavit of eligibility and a liability and publicity release.

5. PRIZE DESCRIPTION: Twenty-five (25) winners and one guest per winner will each receive: (a) an opportunity to participate in a clinic hosted by the Chicago Blackhawks Director of Youth Hockey and a Chicago Blackhawks Alumnus at a Meijer retail store location, as determined by Chicago Blackhawks and Meijer; and (b) a Chicago Blackhawks gift bag. Value of clinic and gift bag is approximately $100.00 per person. Prizes cannot be refunded, substituted, sold or transferred.

6. ELIGIBILITY: Sweepstakes is open only to women who are legal residents of Illinois who are eighteen (18) years of age or older at the time of entry during the Sweepstakes Period, excluding employees of Sponsor, Chicago Blackhawks, and the National Hockey League, and any of their respective affiliate companies, subsidiaries, agents, or advertising agencies, and the immediate family members living in the same household as such employees.

7. GENERAL: Sponsor, the Chicago Blackhawks, the National Hockey League, and their respective agencies and employees, shall have no liability whatsoever for any injuries, losses or damages of any kind caused by any prize or resulting from acceptance and/or use of a prize or participation in this Sweepstakes. No substitution, cash equivalent or transfer of prize(s) permitted except at the Sponsor’s and Chicago Blackhawks’ option for a prize of comparable or greater value. All applicable taxes, and any other expenses not specifically mentioned herein, are the sole responsibility of the winners. No groups, clubs or organizations may participate in this offer or reproduce or distribute any portion of these rules or entry forms to their members. Except where prohibited by law, in consideration for the opportunity to be awarded a prize, entering this Sweepstakes shall constitute (a) a release of Sponsor, the Chicago Blackhawks, and the National Hockey League, and their respective affiliated companies and agents, from any and all liability whatsoever for injuries, losses or damages to person or property of any kind caused by participation in the Sweepstakes and/or acceptance and use of any prize, (b) an agreement to be bound by these rules, and (c) winner’s consent to the use of each winner’s name, address (city and state only), voice and/or image (photograph, video, etc.) for advertising and promotional purposes without further authorization, compensation or remuneration of any kind. Winners need not be present at drawing. Sweepstakes subject to all applicable federal, state and local laws and regulations. Decisions of the Chicago Blackhawks and Sponsor as to all Sweepstakes matters are final. Sweepstakes is governed by the laws of the state of Illinois. Void where prohibited or restricted by law. Winners are responsible for transportation to and from the clinic. Sponsor and the Chicago Blackhawks reserve the right in their sole discretion to disqualify any individual feared to be tampering with the entry process or the operation of the Sweepstakes or to be found acting in violation of the official rules, or to be acting in any manner deemed by Sponsor and/or the Chicago Blackhawks to be unsportsmanlike or disruptive; or with the intent to annoy, harass, abuse or threaten any other person or to undermine legitimate operation of the Sweepstakes.
